We are upgrading our conferencing system (which is the backbone of our already deployed tele emergency doctor system) in terms of reliability/scalability, adding new communication means (video calls, chat), improved logging for troubleshooting/performance measurement/documentation.  Interested in state-of-the-art ways to implement our requirements, looking for partners for the actual implementation.
